All my PCs started to prompt for Name and Initials every time I launch word.

It starts with a little box that says something to the effect of configuring MS word, etc. Then it prompts.

If I put in the name and initials and reopen word it does not prompt again; however, if I reboot the machine(s) it will prompt again.

Office was installed via Group Policy and I am using Deep Freeze (which was in the thawed state) and mandatory profiles. I tried changing them to .DAT and doing it, but it still does not stick.

Anyone run into this?

You probably took a snapshot with Deep Freeze before initialising Office, so everytime Office starts it thinks it's starting up for the first time. If you're going to use mandatory profiles, you probably want to make sure that any annoying initialisation type issues like this are sorted out before taking the snapshot.

With Deep Freeze, I can put the computers in a "thawed" state which will allow me to make changes. Afterwards, I "freeze" the computers again to save the new changes. This works great. The only problem I see now is the installation part.

I wonder if it is in the profile?

That's what I meant. A registry entry in the mandatory profile is still in the uninitialised state. Also, I'm sure this is a per-user setting as well.

This is not the case because I thawed the computers and opened Word.
A registry entry in the mandatory profile is still in the uninitialised state

I tried changing the .MAN to a .DAT and opening word to add those settings. Is there something different I should try?

Come to think of it, I think I also have the "do not allow changes to propagate to server" maybe that's the problem.
